OUR NO SHOW POLICY:
We require a credit card on file to hold your appointment. The card will not be charged if you show up, cancel 24 hours in advance for Monday through Friday appointments. 48 hour cancelation required for the Saturday appointments. If you NO SHOW or FAIL TO CANCEL M-F appointments you will be charged 50% of your treatment fees. Saturday appointments are charged 100% of their normal treatment fee. The minimum fee is $50. JUVEDERM, RADIESSE and BOTOX are charged at full price of the treatment.
There can be no exceptions to this policy without unusual circumstances. Please call us if you have any questions or concerns.

Please provide us with at least 24 hours advance notice for any appointment changes. This will enable us to better accommodate another patient.
We are located near the US 60 and Loop 101 for convenient service to the entire Phoenix area.PRESCRIPTION REFILLS:
Prescription refills are handled during office hours when we have full access to your medical records. Refills are not filled on holidays, weekends, or if you have not been seen for more than six months.
FEES, PAYMENT & INSURANCE:
We do not participate in insurance plans. Payment is required at the time of service.
We do not accept checks! We do accept cash, MC, Visa, credit cards and debit cards.
